Job Summary

The Business Manager plays a vital role in the daily running and development of the Transfer of Care Hub (ToCH), based at Queen Elizabeth Hospital and operating across Lewisham and Greenwich NHS Trust, the Royal Borough of Greenwich, and the London Borough of Bexley.

Key responsibilities include:

  • Managing business coordination, governance, data, and performance functions of the ToCH.

  • Supporting multi-agency teams to ensure safe, efficient, and person-centred discharge across Pathways 1, 2, and 3.

  • Ensuring data and information systems reflect operational practice to guide planning, decisions, and compliance.

  • Acting as a key interface across health, social care, and third-sector partners for reporting, information management, risk tracking, and team coordination.

Main Duties of the Job

1. Business Operations and Coordination

  • Coordinate daily operations of the ToCH, ensuring continuity and effective collaboration.

  • Maintain an operational calendar (daily huddles, board rounds, escalation forums).

  • Manage internal workflows, including handovers, task allocation, trackers, and procedures.

  • Implement and maintain version control of SOPs, forms, and documentation.

2. Performance Monitoring and Analytics

  • Lead on collation, interpretation, and presentation of discharge-related metrics:

    • Criteria to Reside

    • Pathway Utilisation (P1, P2, P3)

    • Length of Stay (LoS)

    • Delayed discharges and reasons

    • SITREP reporting and statutory returns

  • Develop and maintain dashboards and visualisations for stakeholders.

  • Act as lead liaison for data intelligence teams across LGT, Local Authorities, and ICS.

  • Undertake ad hoc data analysis to support decision-making, winter planning, and business cases.

3. Meeting and Governance Support

  • Provide administrative and governance support to discharge forums:

    • Daily ToCH huddles

    • Weekly system escalation meetings

    • Monthly ToCH Improvement Working Group

    • Bi-borough operational discharge coordination

  • Prepare agendas, circulate papers, record minutes, and follow up actions.

  • Maintain governance frameworks, including document repositories and role descriptions.

Person Specification

Essential

  • Degree-level qualification or equivalent experience in healthcare management, business, data, or public administration.

  • Evidence of continuous professional development (CPD).

  • Experience in NHS, Local Authority, or Integrated Care settings.

  • Experience in performance reporting, data analysis, and dashboards.

  • Experience in business management or operational support within clinical/system-facing teams.

  • Ability to lead and deliver projects or service improvement initiatives.

  • Experience supporting or managing multi-agency meetings.

  • Experience supervising staff.

  • Experience writing business cases, SOPs, or guidance documents.

  • Strong understanding of discharge pathways (P1–P3), Home First, and Criteria to Reside.

  • Knowledge of NHS performance targets, dashboards (UEC), and statutory returns (SITREP).

  • Knowledge of partnership working across health, social care, and voluntary sectors.

  • Knowledge of National Delay Reason Codes.

  • Familiarity with financial governance and budget tracking.

  • High proficiency in MS Office and data visualisation tools.

  • Excellent written and verbal communication skills.

  • Strong analytical skills and ability to identify risks and improvements.

  • Ability to manage multiple operational workstreams in a busy environment.

  • Discretion and confidentiality with patient and operational information.

  • Highly organised, proactive, solutions-focused, and resilient.

  • Strong team leadership skills with commitment to diversity.

Desirable

  • Project or performance management qualification (PRINCE2, Lean, QI).

  • Experience using EPR systems, patient trackers, or local discharge databases.
