Livewell Southwest is seeking a registered Social Worker to join its Adult Social Care Care Home Team. This role involves working within a multidisciplinary team, carrying out Assessment and Review functions under the Care Act 2014, with a focus on a strength-based approach to support people in residential and nursing care.

Key responsibilities include:

  • Conducting person-centred assessments and reviews in line with the Care Act

  • Preparing and submitting Court of Protection applications

  • Developing and implementing risk management plans

Main Duties of the Job

  • Undertake complex and proportionate assessments (e.g., mental capacity and health/social care assessments).

  • Arrange and review the delivery of social care support.

  • Support people in complex situations, promoting their outcomes and wishes.

  • Carry out safeguarding investigations and make best-interest decisions, including Deprivation of Liberty considerations.

  • Supervise and train students and Band 5 colleagues when required.

  • Provide supervision (1:1 or peer supervision) to ensure safe practice.

  • Support induction training for new staff.

  • Manage responsibilities such as safeguarding investigations, best-value care planning, and professional risk analysis.

  • Maintain accurate records in line with GDPR and organisational guidelines.

  • Work flexibly across a 7-day service.

Job Description – Responsibilities (continued)

  • Provide short-term security of service users’ belongings (e.g., cash, keys, documents).

  • Plan and organise workload using online systems.

  • Design, implement, and review care/support plans.

  • Complete carers’ assessments and seek feedback from service users.

  • Participate in meetings, case reviews, and multidisciplinary discussions.

  • Apply protocols (e.g., lone working, risk management, health and safety).

  • Advocate on behalf of service users and contribute to service development.

  • Take an asset-based approach, signposting to universal services and community resources.

  • Support complaints processes and apply Duty of Candour principles.

  • Work with autonomy, applying professional judgement within organisational guidelines.

Person Specification

Skills (Essential)

  • Proficient in Microsoft Office (reports, care plans, assessments).

  • Excellent communication and negotiation skills.

  • Strong analytical, observational, and judgemental skills.

  • Ability to prioritise, manage time effectively, and work flexibly.

  • Team player, contributing to wider service objectives.

Knowledge (Essential)

  • National standards and guidance in social work.

  • Policies and procedures of Livewell Southwest.

  • Relevant legislation: Care Act 2014, Mental Capacity Act 2005, Health and Social Care Act 2012, Data Protection, Equality, Human Rights, and related acts.

  • Knowledge of Continuing NHS Healthcare eligibility.

  • Awareness of the impact of unpaid care responsibilities.

Experience

  • Essential:

    • Significant experience as a Social Worker

    • Safeguarding (adults and children)

    • Working with disability, ill health, and crisis situations

    • Care planning, personal budgets, and multidisciplinary working

  • Desirable:

    • Experience within an Adult Safeguarding team

Qualifications

  • Degree or equivalent in Social Work (DipSW, CQSW, CSS).

  • Registration with Social Work England.

  • Ongoing CPD evidence.
