Job Summary
Nottinghamshire Healthcare NHS Foundation Trust is seeking a Social Worker to join the Local Mental Health Team (LMHT) at the City East LMHT.
This is an opportunity to be part of the growing social work workforce within the Trust. Social workers play a vital role in improving mental health outcomes through a human rights approach, strength-based working, and advanced relationship-building skills.
The role involves supporting a caseload of individuals impacted by social issues affecting their mental health. This includes applying social work and psychoeducation interventions, and representing the social perspective in a multi-disciplinary community mental health team. Training, supervision, and opportunities to develop additional skills are provided.

Main Duties
Demonstrate the principles of social work through professional judgement, decision making, and accountability.
Lead on promoting the social perspective and trauma-informed model of mental health within the multi-disciplinary team.
Provide social work expertise to improve mental health outcomes by addressing social determinants of health.
Deliver social work interventions and provide care coordination for individuals facing social stressors.

Job Description – Responsibilities
Maintain an individual caseload and team responsibilities.
Carry out holistic assessments considering health and social needs, including family and carer engagement.
Analyse and manage risk, creating personalised risk and safety plans while balancing safeguarding with self-determination.
Focus on strength- and asset-based approaches to support self-directed change.
Provide expertise in safeguarding, including managing disclosures and information appropriately.
Promote family and carer involvement and collaborate with the Family Intervention Team.
Apply legislation such as the Care Act, Mental Capacity Act, Equality Act, Human Rights Act, Mental Health Act, and related frameworks.
Liaise with Social Care and external agencies to ensure statutory and non-statutory support.
Model anti-discriminatory and anti-racist practice.
Share best practice, promote a learning culture, and supervise colleagues as required.
Occasionally deliver medication in consultation with prescribers.
Undertake other reasonable duties including participation in the duty system.
A full UK driving licence and business-use vehicle is required (reasonable adjustments will be made where applicable).

Person Specification
Qualifications (Essential)
Social Work qualification to degree level or equivalent.
Current registration with Social Work England (SWE).
Ability to maintain professional registration.
Contractual Requirements (Essential)
Full UK driving licence and business-use vehicle (reasonable adjustments for disabilities in line with Equality Act 2010).
