Main Duties of the Job
Champion social care and safeguarding in line with legislation.
Act as an effective team member, offering high-quality services to users and carers.
Conduct assessments, care planning, and coordination from a social perspective.
Deliver recovery-focused care to service-users and carers, ensuring safety and independence.
Follow safeguarding policies, including referrals and child visit requests.
Liaise with families, carers, and support nursing staff in visit coordination.
Ensure involvement of service users and carers in improving services.
Liaise with agencies (Ministry of Justice, Police, Children’s Services, etc.).
Apply the Mental Capacity Act and support colleagues in doing so.
Ensure all records are accurate and up to date.
Champion equality, diversity, and challenge discrimination.
Address adversity and support social inclusion.
Participate in training and post-qualifying development.
Engage in supervision and reflect on personal practice.
Undertake tasks delegated by Hertfordshire County Council.
Adhere to all Trust policies and procedures.
Attend MDT meetings, ward rounds, CPAs, and court hearings when needed.
Carry out any additional duties required by your line manager.

Qualifications (Essential)
Appropriate professional social work qualification
Education to degree level
Registration with Social Work England
Training in outcomes-focused assessments and personalization
Relevant post-qualifying training
Approved Mental Health Professional/Best Interests Assessor training
Qualifications (Desirable)
Training and experience in Safeguarding
Experience (Essential)
Significant post-qualification experience
Strong knowledge of legislation, theory, and care group policy
Experience in personalized assessments and care planning
Understanding of personal budgets and direct payments
Experience in multidisciplinary and collaborative working
Proven partnership work with service users and carers
Experience (Desirable)
Experience as an Approved Mental Health Professional and/or Best Interests Assessor
