Job Summary
Nottinghamshire Healthcare NHS Trust is looking for two social workers to join their Local Mental Health Teams (LMHTs): one based at LMHT Gedling (1.0 FTE) and one at LMHT City East (0.6 FTE).
This is an exciting opportunity to join a growing social work workforce focused on improving mental health outcomes. Social workers will use advanced relationship-based skills, a human rights approach, and strength-based working to support positive, self-directed change.
Successful candidates will work within multi-disciplinary community mental health teams, supporting individuals experiencing social issues impacting their mental health by applying social work and psychoeducation interventions. Social work training and supervision will be provided, along with opportunities to develop additional skills.

Main Duties
Demonstrate social work principles through professional judgement, decision-making, and accountability.
Promote and lead the social perspective and trauma-informed social model of mental health within the multidisciplinary team.
Value and incorporate expertise from service users, carers, and other professionals.
Manage an individual caseload, providing social work expertise to improve mental health outcomes by understanding social determinants of mental health.
Deliver social work interventions and coordinate care for individuals facing social stressors impacting their mental health.
Complete team responsibilities as required.
Conduct holistic assessments, involving individuals’ families and carers when appropriate.
Assess and manage risk, balancing safeguarding with choice, self-determination, and positive risk-taking under relevant legislation.
Support strength- and asset-based working to promote self-directed change and community engagement.
Identify and manage safeguarding issues, including adult and children’s safeguarding responsibilities; act as safeguarding link for the team.
Ensure evidence-based practice informed by social work theories and values, with a human rights approach.
Promote family and carer work within the team, collaborating with the Family Intervention Team.
Maintain expertise on legislation including the Care Act, Mental Capacity Act, Equality Act, Human Rights Act, Mental Health Act, and more.
Work in partnership with external agencies to support access to statutory, independent, and non-statutory services.
Liaise with Social Care to ensure appropriate support levels and help colleagues engage with Social Care processes.
Model anti-discriminatory and anti-racist practice, challenging systemic issues.
Actively participate as a member of the multi-disciplinary team, attending meetings and promoting a learning culture.
Provide supervision to colleagues as required.
Occasionally deliver medication, consulting prescribers and following procedures.
Perform any other reasonable duties, including duty system participation.

Person Specification
Qualifications
Essential:
Degree-level social work qualification or equivalent
Completed ASYE or equivalent
Current registration with Social Work England (SWE)
Desirable:
Completed or willing to work towards Practice Education (PEPS 1 or both)
