Main Duties of the Role

  • Complete assessments for clients and carers

  • Provide care coordination aligned with the Trust’s Care Programme Approach (CPA)

  • Manage a caseload of both organic and functional clients

  • Participate in rostered Duty role and initial CMHT assessments

  • Present assessments during Multi-Disciplinary Team (MDT) meetings

  • Undertake safeguarding procedures with support from Team Manager and Seniors

Team Schedule:
Monday to Friday, standard office hours

Essential Requirements

  • Hold a recognised social work qualification

  • Be registered with the Health & Care Professions Council

  • Access to personal transport for city-wide travel

Person Specification

Qualifications & Training
Essential:

  • DipSW, BA, MA Social Work, CQSW
    Desirable:

  • Practice teachers/assessor qualification

Additional Qualifications
Essential:

  • AMHP status or willingness to undertake training

Knowledge & Experience
Essential:

  • Experience in health and social care assessments

  • Experience commissioning care packages
    Desirable:

  • Experience with older adults with complex needs

  • Experience in safeguarding investigations

Skills & Abilities
Essential:

  • Skilled in managing workload and making informed decisions

  • Sensitive assessment of vulnerable individuals and families

  • Ability to write reports that meet legal requirements

  • Strong time management and flexibility
