Job Summary
The team is based at Hall Court in Telford and provides mental health assessment, treatment, and support for people in Telford & Wrekin. As a key part of the Urgent Care Pathway, the team operates 24/7, delivering support to individuals experiencing mental health crises or requiring enhanced support at home.
The multidisciplinary team includes Band 5 and 6 Nurses, Social Workers, Band 3 Support Workers, Non-Medical Prescribers, Medical Staff, Occupational Therapists, Administrators, and Business Support Staff. The diversity of roles enables the team to respond effectively to a broad range of mental health needs.
We are seeking a Social Worker responsible for delivering planned, recovery-focused care. This role involves working in partnership with service users, carers, internal teams, and external agencies to ensure seamless care within defined pathways and clusters.

Main Duties of the Role
As part of a multi-disciplinary team and in collaboration with internal and external agencies, the Social Worker will:
Provide specialist assessment and care planning with service users and carers
Evaluate treatment plans and provide home treatment
Triage and assess incoming referrals in liaison with colleagues
Coordinate the work of the team

Essential: First Degree Level qualification / DIPSW Practice
Experience
Essential: Postgraduate-level experience in mental health settings delivering and managing care
Desirable: Experience in a community mental health setting
