Job Summary
The Community Rehabilitation Team is seeking an experienced Social Worker to join its multidisciplinary team.
The role involves working with people with relapsing and remitting mental ill health, supporting transitions from inpatient rehabilitation facilities to community living, and helping service users improve their quality of life.
Key aspects:
Focus on the Recovery Model with a strengths-based and trauma-informed approach
Support independence, autonomy, and wellbeing
Collaborate with local authorities, inpatient services, and community care providers
Work trust-wide across five boroughs of Pennine Care (driving essential)
Main Duties of the Job
Manage a caseload of service users in the Community Rehabilitation Team
Provide specialist social work interventions to service users and their families/carers
Guide the team on social care knowledge, legal literacy, and local authority systems
Make referrals to Local Authorities when required
Champion the social model of care while applying a biopsychosocial approach
Contribute to risk assessments and management plans, including hospital-to-community transitions
Promote positive risk-taking, choice, and control
Hours: Monday to Friday, 9am–5pm

Qualifications
Essential:
Social Work Qualification (BA, MA, MSW, DSW, CQSW)
Postgraduate-level courses/experience (e.g., ASYE)
Registration with Social Work England
Experience
Essential:
Post-registration experience with complex mental health needs
Experience in multi-disciplinary settings
Experience with community-based care providers
Developing trauma-informed, collaborative risk management plans (including self-neglect, domestic abuse, substance use, risk to others)
Desirable:
Inpatient experience within a Low Secure Service
Forensic setting experience
Knowledge
Essential:
Applied knowledge of Mental Health Act, safeguarding (adults & children), mental disorder, and professional code of conduct
Understanding social determinants of health and wellbeing
Range of appropriate interventions
Governance and MHA knowledge
Desirable:
Knowledge of NHS systems, e.g., Data Security and Protection Toolkit
