Main Duties of the Job
The role requires carrying out social work duties in a fast-paced multi-disciplinary team, ensuring effective and safe integrated services for adults. Responsibilities include:
Assessing, planning and reviewing in line with the Care Act (2014) and s117 of the Mental Health Act (1983).
Taking a strengths-based approach to support.
Managing a caseload collaboratively to develop assessments, recovery plans and structured care plans.
Signposting to non-statutory services or commissioning care packages.
Completing high-quality Mental Capacity assessments (in line with the Mental Capacity Act 2005) and chairing Best Interest Meetings.
Making safeguarding enquiries in line with s42 of the Care Act (2014).
Complying with SWE Professional Standards and being registered with Social Work England.
Following legislation, national and local policies, and procedures.

Person Specification
Education / Qualifications
Essential: Professional Qualification in Social Work (BA, MA, or equivalent), Registered with Social Work England.
Desirable: Practice Educator, AMHP, BIA.
Experience
Essential: Experience working within a multi-disciplinary team, completing assessments under key legislation, and producing collaborative care plans.
Desirable: Completing s42 enquiries and Mental Capacity Act assessments.
Knowledge
Essential: Strong knowledge of Care Act (2014), Mental Capacity Act (2005), Mental Health Act (1983), Human Rights Act (1998), relevant case law, and policy.
Knowledge of risk assessment and formulation.
Skills and Abilities
Essential: Strengths-based and recovery-focused approach, effective communication, co-production of care plans, and reflective practice.
Desirable: Ability to write reports suitable for court.
Other Requirements
Essential: Frequent travel within CMHT areas and occasional travel across the Trust.
