Job Summary

The team is based at Hall Court in Telford and provides mental health assessment, treatment, and support to people in the Telford & Wrekin area.

As an integral part of the Urgent Care Pathway, the team works 24 hours a day, 7 days a week, supporting people experiencing mental health crises, those needing additional support alongside existing networks, or those receiving treatment at home equivalent to what would be provided in a Mental Health Hospital.

The team includes Band 5 and 6 Nurses, Social Workers, Band 3 Support Workers, Non-Medical Prescribers, Medical Staff, Occupational Therapists, Administrators, and Business Support Staff.

The Trust is recruiting a Social Worker to:

  • Oversee and deliver recovery-focused care in partnership with service-users, carers, and other professionals.

  • Work collaboratively with other services to provide seamless care along defined care pathways and clusters.

  • Partner with external agencies to support service-users in accessing independent and non-statutory sector services.

Main Duties of the Job

As part of a multi-disciplinary team, working with internal and external agencies, the post holder will:

  • Provide specialist assessment and care planning with service-users and carers.

  • Evaluate treatment plans and deliver home treatment.

  • Triage and assess referrals in liaison with multi-disciplinary colleagues.

  • Co-ordinate the work of the team.

Qualifications

Essential

  • First Degree Level qualification / DIPSW Practice

Experience

Essential

  • Post-graduate level practice experience in delivering/managing care and risks in mental health settings.

Desirable

  • Experience in a community mental health setting.
