As a Nurse with TLC Care, you are responsible and accountable for the wellbeing of residents within the care home, ensuring their physical, emotional, and social needs are met. The role involves providing leadership to the care team while assessing, developing, and evaluating individualised resident care in consultation with residents, GPs, relatives, and carers.

Main Purpose
Achieve the highest possible standard of care through direct nursing care and effective supervision of employees.
Take management responsibility for a shift as directed.
Main Responsibilities
Care and Care Planning
Report any ill health amongst residents and request GP/Professional visits where necessary.
Ensure Care Plans are completed and maintained with residents, relatives, and healthcare professionals, in line with NMC guidelines and company policy.
Assess all aspects of residents’ care needs and provide health supervision and direct Nursing Care when required.
Conduct ongoing assessment and review of all residents’ social and healthcare needs.
Maintain accurate records and ensure each resident has an individualised Care Plan.
Administer prescribed medicines and document in accordance with NMC guidelines.
Act as the named Nurse for a number of residents.
Tissue Viability
Ensure no home-acquired pressure sores through delivery of high standards of care.
Report and document any pressure sores to the appropriate parties.
Develop individualised care plans for residents at risk.
Nutrition
Complete monthly MUST assessments.
Develop care plans for residents at risk, with detailed interventions.
Maintain fluid and food intake charts for residents with nutrition/hydration issues.
Record dietary requirements in care plans and communicate with the kitchen.
Ensure PEG feeding residents have appropriate care plans in place.
Continence
Complete continence assessments where required.
Ensure all residents with catheters have individualised catheter care plans.
Infection Control
Uphold and promote the Home’s infection control policy.
Monitor staff adherence to universal precautions (hand hygiene, gloves, aprons, etc.).
Resident Hygiene
Ensure residents are well groomed, appropriately dressed, and that care records reflect resident choices.
Resident Mobility
Encourage independent movement and ensure accessibility of walking aids.
Ensure bedrails are not routinely used.
Develop care plans for residents at risk of falls.
Dignity and Respect
Ensure residents are addressed by their preferred name and treated respectfully.
Monitor staff to ensure they knock before entering residents’ rooms and engage personally.
Communication
Participate in staff, resident, and relative meetings as required.
Maintain good communication with residents, relatives, and the multidisciplinary team.
Handle complaints effectively.
Training & Development
Mentor student nurses and carers.
Maintain and improve professional knowledge and competence.
Stay updated in clinical expertise.
Attend mandatory training as required.
Health & Safety
Understand and implement the Care Home’s Health and Safety, Emergency, and Fire procedures.
Promote safe working practices.
General
Adhere to NMC guidelines and regulations.
Maintain records as required by the Registering Authority and General Manager.
Skills, Knowledge, and Qualifications
NMC Registered Nurse with a valid NMC PIN Number.
Ability to motivate and delegate effectively.
Strong communication skills at all levels.
Genuine interest in working with the elderly.
Satisfactory DBS and DBS Adult First check.
Team player with the ability to work across a multidisciplinary team.
