Job Summary

Cheshire and Wirral Partnership NHS Foundation Trust (CWP) has a vacancy for a qualified Social Worker within the Integrated Services for Adults with Disabilities, based at the Millennium Centre in Leasowe, Wirral.

The team works with people with complex needs, supporting them and their carers to achieve the outcomes they require. CWP is at the forefront of health and social care integration, providing innovative approaches to support delivery.

Applicants must be person-centred practitioners with experience in:

  • Care Act assessments

  • Developing personalised support plans

  • Completing mental capacity assessments

  • Leading safeguarding enquiries

A good understanding of asset and strength-based approaches is essential.

Applications are welcomed from newly qualified staff or final-year social work students (salary differs from Band 6 in these cases).

Person Specification

Qualifications (Essential)

  • Social Work qualification (Degree in Social Work; DipSW or equivalent)

  • Registered with Social Work England (SWE)

  • Evidence of CPD (Professional Capabilities Framework)

  • Willingness to undertake the Practice Educator Award

Desirable: Relevant post-qualifying/vocational training

Knowledge & Expertise (Essential)

  • Knowledge of relevant legislation: Mental Health Act 1983, Mental Capacity Act 2005, Care Act 2014, Disability Discrimination Act 1995, Safeguarding

  • Understanding of confidentiality and data protection

  • Strong verbal and written communication skills

  • Awareness of current challenges in social work

  • Organisational skills and IT proficiency

Desirable: Emotional intelligence and resilience

Experience (Essential)

  • Working with carers and vulnerable individuals/families

  • Developing support plans or care packages

  • Multi-agency partnership working
