Job Summary
All Social Workers must be registered with Social Work England.
The Social Worker role involves working as part of a multidisciplinary team to deliver high-quality social care services to adults experiencing mental ill-health. The position focuses on assessment, care planning, and intervention from a social perspective for adults accessing services.
The role supports recovery-focused care that promotes choice, control, and independence for service users and carers while ensuring safety. The Social Worker will act as a champion of social care and safeguarding within their locality.
Main Duties of the Job
The Social Worker will:
Assess needs and outcomes and analyse complex information related to service users and carers.
Provide professional recommendations and maintain accountability for social care decisions.
Engage service users and carers to learn from their experiences and improve service delivery.
Undertake surveys and audits within their locality and contribute to trust-wide surveys when required.
Deliver social care assessments and commissioned packages of care in line with the Care Act 2014, ensuring statutory social care functions are met.

Key Relationships
The Social Worker will:
Work as part of a multidisciplinary team.
Collaborate with service users, carers, and external stakeholders within and outside the Trust.
Demonstrate dignity and respect, focusing on personalisation, choice, and control.
Act as a champion and expert in social care and safeguarding, ensuring colleagues deliver high-quality social care services.
Duties and Responsibilities
Champion social care with a strong understanding of legislation and statutory guidance.
Lead adult safeguarding practices, undertake enquiries, and contribute to care plans protecting both adults and children.
Conduct comprehensive assessments and reviews for individuals presenting with high-risk or complex needs, in line with the Care Act 2014.
Provide direct support, advice, and information to service users and carers.
Ensure appropriate health and social care services are commissioned, including direct payments where applicable.
Contribute to core tasks within EPMHS, including Dialog+ First Conversations.
Support service users to live independently within the community with access to appropriate support services.
Undertake regular risk assessments to safeguard vulnerable individuals.
Complete carer assessments, develop contingency plans, and offer appropriate support.
Build partnerships to promote recovery-focused community services and best practice.
Ensure service user and carer feedback informs service improvement.
Maintain accurate and up-to-date electronic records.
Undertake ongoing training and maintain registration with Social Work England.
Provide management and reflective supervision to newly qualified social workers and unregistered staff.
Support newly qualified social workers through the Assessed and Supported Year in Employment (ASYE).
Undertake or be willing to train as an Approved Mental Health Professional (AMHP).
Participate in regular supervision and reflective practice.
Fulfil delegated social care responsibilities on behalf of Hertfordshire County Council.
Comply with all Trust policies and procedures.
Undertake additional training as required.
Carry out other relevant duties as assigned by the line manager.

Person Specification
Qualifications / Education / Training
Essential:
Professional Social Work qualification.
Degree-level education.
Registration with Social Work England.
Training in outcomes-focused assessments and personalisation.
Relevant post-qualifying training.
Approved Mental Health Professional (AMHP) or Best Interests Assessor (BIA) training.
Training and experience in safeguarding.
Knowledge, Training, and Experience
Essential:
Minimum of two years’ post-qualification experience.
Knowledge of legislation, theory, and policy relevant to care groups.
Experience in assessments, personalisation, and care planning.
Understanding of personal budgets and direct payments.
Experience of multidisciplinary and partnership working.
Experience collaborating with service users and carers with dignity and respect.
Desirable:
Experience as an AMHP and/or BIA.
Experience in integrated health and social care environments.
Disclosure and Barring Service (DBS) Check
This position falls under the Rehabilitation of Offenders Act (Exceptions Order) 1975, requiring a Disclosure and Barring Service (DBS) check for any previous criminal convictions.
