Main Duties of the Job

The Social Worker will be responsible for:

  • Assessment, planning, implementation, and evaluation of evidence-based interventions.

  • Managing a caseload of service users within a multidisciplinary team and Care Programme Approach.

  • Acting as a care coordinator with a specialist focus on release planning.

  • Supporting individuals from assessment through to discharge, including care transfers to other providers such as hospitals or CMHTs.

  • Collaborating with various agencies across the Offender Health Pathway.

  • Rehabilitating and supporting service users to reintegrate successfully into the community.

  • Working with individuals with complex needs, including learning disabilities, mental health issues, and forensic or offending histories.

The role offers the chance to work alongside a supportive and experienced team within Inclusion, part of Midlands Partnership NHS Foundation Trust (MPFT). The position also includes access to professional development and training opportunities offered by the NHS.

Person Specification

Qualifications and Training

Essential:

  • Professional Social Work qualification (DipSW, CQSW, CSS) or equivalent foreign qualification certified by the Health and Care Professions Council (HCPC).

Experience

Essential:

  • Post-registration experience in mental health services.

  • Prison experience is desirable.

Disclosure and Barring Service (DBS) Check

This position is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, requiring a submission for Disclosure to the Disclosure and Barring Service (DBS) to check for any prior criminal convictions.

Certificate of Sponsorship
