Main Duties of the Job

Follow patients through their full journey, from hospital admission to discharge and reintegration into the community.
Work four days per week, including one 8AM-8PM shift.
Carry a small, focused caseload to maintain quality and continuity of care.
Apply your professional skills in complex ethical and legislative contexts.

The post is a permanent contract, full time at 37hrs per week.
Salary: £45,091 – £49,282 (dependant on experience) including annual increments.
