Main Duties of the Job

  • Serve as a core member of the multi-disciplinary Baywide Social Care Team.

  • Contribute to assessment and discharge planning for working-age adults in Torbay and regional hospitals.

  • Undertake holistic social care assessments and prepare care plans in partnership with service users and carers.

  • Commission and monitor services to ensure cost-effective, high-quality care packages.

  • Safeguard adults and children while adhering to Human Rights legislation.

  • Comply with social care legislation, including the Care Act 2014 and Mental Capacity Act 2005.

Job Description and Responsibilities

Communication and Relationships

  • Work with individuals experiencing complex emotional or psychological conditions.

  • Manage communications requiring negotiation, counselling, and reassurance.

Planning and Organisation

  • Prioritise caseloads and manage time effectively.

  • Develop detailed care plans with service users and carers.

  • Participate in referral allocation meetings.

Responsibility and Accountability

  • Manage a defined caseload and provide statutory social work services.

  • Conduct high-quality assessments and reviews under the Care Act 2014.

  • Promote co-designed support plans with measurable outcomes.

  • Engage with relevant individuals and agencies during assessments and reviews.

  • Advocate for vulnerable individuals and lead safeguarding enquiries when appropriate.

  • Work within the Care Act Safeguarding Adults framework and relevant Trust policies.

Policy and Service Responsibility

  • Complete Care Act assessments and maintain records per service procedures.

  • Collaborate within an integrated health and social care team.

  • Contribute to service development, particularly in the hospital discharge setting.

Finance, Equipment, and Resources

  • Collaborate with community and voluntary organisations to identify suitable support providers.

  • Report any service provision deficiencies to the Social Care Lead.

Supervision, Leadership, and Management

  • Participate in regular supervision and agree on training and development plans with a line manager.

  • Attend relevant courses and appraisals to enhance knowledge and skills.

Information Technology and Administration

  • Maintain accurate and up-to-date records.

  • Complete social care panel paperwork as required.

Person Specification

Qualifications and Training

Essential:

  • Degree/Diploma in Social Work or equivalent qualification.

Desirable:

  • Post-qualifying consolidation and commitment to ongoing training.

Knowledge and Experience

Essential:

  • Knowledge of safeguarding processes, Care Act, and Mental Capacity Act.

  • Experience in multi-disciplinary collaboration.

  • Strong communication, time management, and IT skills.

Desirable:

  • Knowledge of Health and Safety legislation, Continuing Care, Direct Payments, and end-of-life care.

Specific Skills

Essential:

  • Ability to work as part of a multi-disciplinary team and meet deadlines.

  • Strong recording, time management, and problem-solving skills.

Desirable:

  • Supervision, presentation, and group work skills.

Work Environment Requirements

Essential:

  • Ability to travel across geographical areas.

  • Willingness to visit clients in various settings, including hospitals, residential homes, and community environments.

Disclosure and Barring Service Check

This role is exempt under the Rehabilitation of Offenders Act (Exceptions Order) 1975, requiring a Disclosure and Barring Service (DBS) check for previous criminal convictions.
